$50 NEW PATIENT REGISTRATION FEE

This charge will be applied to hold your 90 minute new patient appointment.

If you cancel appt at a later date, this will not be refunded. You will not be charged again if you need to reschedule.

If you un-enroll and wish to re-enroll after 30 days, there is a $150 re-enrollment fee.
